New Patients – Itasca, IL
We Are Ready to Welcome You!
Welcome to Hamilton Lakes Dentistry! We are currently accepting new patients and look forward to providing exceptional dental services that are designed to improve, enhance, repair, and restore all types of smiles. Our salon-like feel will help you experience a more relaxed visit, while our dental team works to provide top-of-the-line treatment that is second to none. But before you arrive, we invite you to review the following information and call us if you have any questions.
Your First Visit
When visiting for a comprehensive exam, you will spend about 1.5 hours with our team. During this visit, we will perform a pre-clinical examination, complete with diagnostic imaging, probing, and a 60-point new patient exam. We’ll also discuss any potential findings that we come across and formulate a customized treatment plan for you to review.
If arriving for a dental hygiene exam, you will spend 1.5 hours with our hygienist to undergo a full mouth probing and limited intraoral photos. Then, we will perform a 3-point exam that allows us to identify and treat a particular area. We may also find it beneficial for you to return for a comprehensive exam.
Dental Patient Forms
There is no need to spend additional time in our lobby filling out new patient paperwork when you can do it from the comfort of your home. Simply click the link below to download the available forms and make sure to bring them with you (completed) for a smoother check-in process.
At Hamilton Lakes Dentistry, we refuse to let insurance companies decide what is best for you; therefore, we are not tied to any insurance network. However, being an out-of-network provider doesn't mean we won't work with your insurance company! We still want you to receive the maximum reimbursement possible and are happy to file claims with your insurance, just like a typical dental office. This allows us to see one patient at a time, use high quality materials, and give you unbiased recommendations.
To assist in lowering out-of-pocket costs, we are pleased to offer flexible financing. We offer short term in-office payment plans, as well as long term payments via CareCredit. CareCredit is what we work with to help you keep more money in your pocket and remain within your budget while working toward a healthier, more beautiful smile. If you’re interested in learning more or applying, call our dental office and ask a member of our team.
Instead of wasting time mailing in a check or calling to speak to a member of our team, you can pay your bill online quickly and easily. Just click the link below to be redirected so that you can avoid unnecessary phone calls or a trip to the post office just to pay your bill.
